Health Care Transparency
We support transparency in health care costs and believe employees should have access to information that helps them make informed decisions about their care.
Our medical plan is administered by UMR, a UnitedHealthcare company. UnitedHealthcare, UMR, and HealthSCOPE Benefits publish Machine-Readable Files on behalf of the Sempreon Health and Welfare Plan. These files contain pricing information for covered health care items and services and may be used by researchers, developers, and other organizations to support consumer education and plan comparison efforts.
To access the Machine-Readable Files, please visit the Transparency in Coverage website.
Form 1095-C Information
As an Applicable Large Employer, Sempreon provides Form 1095-C annually to eligible employees. This form contains information regarding health coverage that was offered during the year, as well as any covered dependents.
If you need a paper or electronic copy of your Form 1095-C, please contact the Human Resources Department.
Understanding Price Transparency
Health plan price transparency initiatives are intended to give consumers greater visibility into the cost of health care services before receiving treatment. By making pricing information more accessible, employees can compare providers, understand potential costs, and make informed health care decisions.
